Please print and return our donor card with your donation, or you may mail a check payable to the PSD Foundation to 1630 S. Stover St., Fort Collins, 80525. Please indicate the area in which you'd like your funds to be used:

• Area of Greatest Need (determined by the Board of Directors)
• Innovative Programs Fund (includes Supporting Partnerships in Innovative Education - SPIE grants)
• Operations Fund
• Endowment Fund

PSD employees wishing to donate by payroll deduction may contact the PSD Foundation office at (970) 490-3292 to request a payroll deduction form. Donations made by payroll deduction are tax deductible.
